FDA to Limit Nicotine Levels in Cigarettes

Last month, the Food and Drug Administration (FDA) took a pivotal first step to reduce nicotine levels in combustible cigarettes by publishing an advance notice of proposed rulemaking (ANPRM) to initiate a public dialogue regarding its plan to implement a product standard to set a maximum nicotine level in traditional cigarettes to make them minimally addictive or non-addictive.

Smoking is the leading cause of preventable death and disease in the US

Cigarettes are responsible for over 480,000 deaths in the US each year, more than “AIDS, alcohol, illegal drug use, homicide, suicide, and motor vehicle crashes combined.” In fact, smoking costs close to $300 billion in direct health care and lost productivity per year.

Tackling the addictiveness of cigarettes by reducing the level of nicotine in them has the potential to positively impact the success rates of those trying to quit the deadly habit while also preventing youth experimenting with cigarettes from getting hooked.

Included in the ANPRM are new estimates published in the New England Journal of Medicine that evaluate one possible scenario that, if implemented, could help about 5 million additional adults quit within one year of implementation. By the end of the century, that same model could reduce the smoking rate from 15 percent to just 1.4 percent, saving more than 8 million lives.

Key difference between combustible and non-combustible nicotine delivery methods

There is no denying that nicotine is an addictive substance; however, when delivered through combustible cigarettes, users also inhale other toxic chemicals, including an assortment of carcinogens. Not only do smokers expose themselves to toxins such as hydrogen cyanide, arsenic, and carbon monoxide, but the tobacco smoke emitted also adversely affects non-smokers around them.

American Cancer Society Adopts New Position on Electronic Cigarettes

The American Cancer Society’s (ACS) recent position statement on e-cigarettes reveals a notable and encouraging change from its previous stance on electronic nicotine delivery systems (ENDS). Noting that “Combustible tobacco products, primarily cigarettes, are the single greatest cause of cancer and kill about 7 million people worldwide each year,” the society now supports the use of e-cigarettes as a tool to help smokers quit the lethal habit.

Why did the ACS change their position?

As a result of the growing body of evidence confirming that the current generation of e-cigarettes is less harmful than traditional cigarettes, many government agencies have revised their positions on these electronic devices, used by millions of consumers to help them quit smoking. In July 2017, the US Food and Drug Administration (FDA) announced a similar shift in policy, and it is now even considering regulating ENDs as an over-the-counter drug. In the UK, the National Health Service (NHS) has already begun to promote them as a safer nicotine delivery alternative, alongside the nicotine patch, gum, and lozenges. It is not surprising, then, that the ACS has also altered its position to reflect the scientific community’s new findings and, ultimately, to better support clinical and policy recommendations that will aid smokers who want to quit.

While the ACS advises clinicians to continue to recommend FDA-approved cessation tools to smokers, it recognizes that e-cigarettes may be the preferred method to achieve smoking cessation goals for many.

Although traditional nicotine delivery alternatives provide smokers with the drug their body craves, alleviating the often painful physical withdrawal symptoms, they fail to satisfy the critical psychological and emotional aspects that can make quitting even more challenging. Is Vaping Safer Than Smoking? NYU Research Suggests It Is

Earlier this year, New York University (NYU) researchers released the findings of their report titled “Harm Minimization and Tobacco Control: Reframing Societal Views of Nicotine Use to Rapidly Save Lives,” which suggests that vaping is much less harmful than smoking cigarettes.

Harm Minimization

The harm reduction approach acknowledges that the adverse effects of nicotine-containing products fall on a continuum of harm, ranging from exceptionally low to exceptionally high. So, is vaping safer than smoking? According to the report, non-combustible nicotine-containing options such as patches, gums, nasal sprays, and e-cigarettes fall on the lower end of the spectrum while combustible nicotine delivery systems such as traditional cigarettes, pipes, and cigars fall on the higher end, meaning that switching from smoking to vaping would result in significantly less harm to the user.

Citing a previous study published in the British Medical Journal (BMJ), a leading medical journal, they assert that the “replacement of most cigarette use by e-cigarette use over a 10-year period yields up to 6.6 million fewer premature deaths with 86.7 million fewer life years lost.”

With that in mind, they posit that any reduction in relative harm from a previous level should be encouraged. Although the best case scenario is one where users don’t use any nicotine-containing products at all, they promote a more realistic view whereby if the consumer does not want to stop the consumption of nicotine, then the goal should be to eliminate their use of smoked or combustible tobacco. In other words, they should be encouraged to use alternative non-combustible products like the ones mentioned above.

Why Are E-Cigarettes the “Sweet Spot?”

While combusted products such as cigarettes score high on appeal and satisfaction, they are also the most toxic. While nicotine replacement therapies such as gums, patches, and nasal sprays provide nicotine without the deleterious toxins present in smoke, they lack the appeal and satisfaction associated with the inhaling and exhaling of vapor and are therefore not likely to replace cigarettes at a population level. Cancer Research UK Launches E-Cigarette Campaign in Greater Manchester

Cancer Research UK, in collaboration with Public Health England (PHE) and the Greater Manchester Health and Social Care Partnership, has recently launched an e-cigarette campaign with the aim to increase awareness of the relative safety of these electronic devices compared to smoking combustible cigarettes.

Since Manchester has a high smoking prevalence level, the campaign is targeting local smokers aged 25 to 55 who are looking to quit the deadly habit. The marketing initiatives include outdoor advertising, ads on buses, billboards, as well as phone booth and washroom posters, which can be seen across Greater Manchester until February 18th.

According to research published by Action on Smoking and Health (ASH), “in 2017 only 13% of adults correctly identified that e-cigarettes are much less harmful, compared to 21% in 2013.” Since there is rampant, and increasing, misconception among the general public that e-cigs are just as, if not more, damaging than traditional cigarettes, the campaign strives to acquaint people with the research to date. According to Cancer Research UK, the evidence shows that vaping is much safer than smoking as “there is no smoke, tar or carbon monoxide, and studies looking at key toxicants have generally found much lower levels than in cigarettes.”

Another key goal of the campaign is to inform people that e-cigarettes do not contain cancer-causing tobacco, although they may contain nicotine. However, despite the substance being addictive, the delivery mechanism – heating as opposed to burning – makes e-cigarettes “far closer to other nicotine replacement therapy (NRT) products than tobacco in terms of harm.”

Lastly, they want to highlight that many smokers in the UK are turning to e-cigarettes to help them stop smoking. In fact, for the first time, a survey conducted by ASH found that there were more ex-smokers, 1.5 million, who use the electronic devices than current smokers. Furthermore, “the main reason people offered for their use of e-cigarettes was to stop smoking.”
